What personal data I collect from you and why I collect it
Booking System
When making an appointment, I will require your name, telephone number and email address in order to confirm your appointment and be able to contact you should the appointment need to be amended or cancelled for any reason.
Pre-Treatment Questionnaire
If it is your first appointment with me, I shall require you to fill in a pre-treatment questionnaire. These questions ask for specific information to ensure I can provide the best possible treatment plan for you.
Covid-19 Pre-Treatment Questionnaire
It is vitally important that you answer a specific Covid-19 related pre-treatment questionnaire 48 hours before attending your appointment. This will be emailed to you and will ask about your current state of health. I cannot treat you without this information.
Who I share your data with and how long it is retained
In accordance with my massage insurance company and massage governing body – The Federation of Holistic Therapists, I am to keep your information on file for 7 years.
I will not share your information with any third parties unless written consent is given by you – and then, it will only be in relation to referring you for further treatment with a physiotherapist/osteopath/chiropractor or medical practitioner.
What rights you have over your data
You can request to view any information I hold on you by contacting me via email.
How I protect your data
Your information is stored on my booking system – Acuity Scheduling, which is GDPR and HIPAA compliant. Any inactive clients will be deleted from the system, although any written treatment notes will be kept in a locked filing cabinet for 7 years (as per my insurance dictates). The Covid-19 pre-treatment questionnaire is provided by Jot Form and is GDPR compliant. These forms will be deleted 14 days after your treatment.
